New Patient Information

How to request a New Patient Appointment 

Please send your request by email to endocrinemd@icloud.com and the reason for your visit.

After receiving your request, we will respond by email to schedule your appointment.

We will provide you with your patient portal login information, new patient paperwork, and instructions on how to upload medical records.

Traditional Medicare
          (annual concierge service fee $1000 + tax)

Our concierge fee is due within 30 days of the initial consultation, and annually thereafter. This fee is non refundable. This is separate from fees charged for individual visits which are covered by Medicare. Unfortunately, this fee is not reimbursable by Medicare.

Our office will submit claims for patients with traditional Medicare Part B. Patients are responsible for the remaining balance after claims have been processed. For more information, please view Insurance & Reimbursement tab or click here.

Commercial Insurance & Medicare Advantage Plans

Our concierge fee is due within 30 days of the initial consultation, and annually thereafter. This fee is non refundable.

Some commercial PPO insurance with out-of-network benefits may reimburses you for services. Click here for more information. We recommend contacting your insurance prior to requesting an appointment.

Medicaid & Managed Medicaid Plans

Currently Dr. Sloan is unable to provide care to anyone with Medicaid. We are not in-network at this time. Medicaid will NOT pay for office visits, lab tests, imaging or medication ordered by Dr. Sloan until future notice.
